It is now recognised that health and social care outcomes are interdependent and as a result Integrated Care has become one of the key aspects of health and social care reform in the UK. By closing the gap between health and social care the government and health service is aiming to manage the ever-increasing demands placed on care services more effectively.
The Integrated Care Programme is a national communication platform, produced by Sovereign Publications in association with the National Association Of Primary Care (NAPC). The Integrated Care Programme is specifically designed to help primary, community and secondary care professionals to work on a more collaborative basis with local authorities and other sectors, including social care to develop effective integrated care organisations that provide more personal, responsive care and deliver better outcomes for a local population.

As a result of its’ ongoing work within the health and social care sector, over the past several years, Sovereign Publications has developed a comprehensive database of primary, community and secondary care professionals, local authorities and other sectors, including social care, who are directly responsible for the ongoing development of Integrated Care Organisations. NATIONAL ASSOCIATION OF PRIMARY CARE (NAPC)
The National Association Of Primary Care (NAPC) was formed as the result of a merger between the National Association of Fundholding Practices (NAFP) and the Association of Independent Multifunds (AIM). The organisation, both in its original guise and in its current role, offers support and advice to its members and also holds routine discussions with Ministers for Health and their officials, with a view to influencing policy on the future direction of primary care.
The NAPC believes the future of primary care lies in service redesign, public and patient engagement as well as in the Practice Based Commissioning scheme, which it enthusiastically supports. The Association is heavily involved in meetings with Government to assist in removing some of the barriers to the operation of the scheme, brought about by the reconfiguration of Primary Care Trusts and some of its personalities.
The Integrated Care Programme is a major platform of communication and forms an important part of the Association's ongoing work in informing, exchanging ideas and uniting medical experts in order to raise the standard of healthcare in the UK. The book is specifically designed to provide decision-makers within health and social care with relevant information on the latest developments affecting ICOs.
